Getting Started: Form A- This is a monthly form that should be faxed to your behavior coach each month.

Gotchas- This is a gotcha tracking system developed by Lynn Hooper at Madison Place Elementary in Olathe, Kansas. Each teacher has their own copy of the file with their own students' names. This allows the teacher to see who is lacking in "gotchas". You need to have an Microsoft Excel to use this program.
